Required medical questionnaire issued by Columbia Pictures before production of Coppola’s 1992 film Bram Stoker's Dracula, which the director has marked and annotated in black ink with his birth date and the name of his physician and address. Coppola checks all of the “NO” boxes for any health problems, writes “No” to two questions below, and ticks the “YES” box affirming that he is in good health. Signed at the conclusion by Coppola. In fine condition.
