A State Athletic Commission application card for a boxer license renewal, partially filled out by Ortiz in ballpoint, listing his given name, “Carlos Ortiz,” address, ring name (“Carlos Ortiz”), ring weight (“135”), birth date, color (“W”), and manager, “Ed Ferguson.” Signed again at the conclusion by Ortiz. The reverse, which bears a stapled glossy photo of Ortiz, notes that his license was restored on October 1, 1958. In fine condition, with a central vertical fold. Less than a year later, Ortiz would win his first professional boxing titles as a light welterweight—NYSAC and NBA—when he defeated Kenny Lane via TKO at Madison Square Garden on June 12, 1959.
