A State of California Agriculture and Services Agency document for a referee license renewal, with some of the upper fields filled out in blue ballpoint by Archie Moore, who writes his full name (“Moore Lee Archie”), street number (“3517 ‘E’ St”), birth date (“12-13 [19]16”), age (“56”), occupation (“tech adviser of tactics”), and writes “no” when asked if he’s been convicted of a felony or misdemeanor since the granting of his former license. He checks the ‘No’ boxes for the remaining questions and signs at the conclusion, “Archie Lee Moore, 1-27-73.” The upper left corner bears an affixed glossy 2 x 2 photo of Moore, and the balance of the application is filled out in another hand. In fine condition, with a light crease across the small photo.
