Yastrzemski, 24 Chatham Way, Lynnfield, Mass.," indicating a withholding exemption of "4," signed at the conclusion by Yastrzemski. In fine condition, with a Beckett label affixed to the reverse. Accompanied by a certificate of authenticity from Beckett Authentication Services. At the time he filled out this document, Yaz was heading into his fifth season with the Boston Red Sox—one in which he was named an American League All-Star and won a Gold Glove.
