Twentieth Century Fox unemployment insurance and old age annuity questionnaire, filled out and signed by Hawks, answering about a dozen questions regarding his age and residence: Hakws makes note of his date and place of birth ("May 30, 1896, Goshen, Indiana"), current age ("39"), and length of residency in California ("25 years").
