"Flown in Lunar Orbit for 6 days during Apollo 15"—CDR Scott's 'CSM Contingency Checklist,' a resource of "paramount importance"

Dave Scott’s complete ring-bound "CSM Contingency Checklist" that was carried into lunar orbit during the Apollo 15 mission, 6 x 8, signed and flight-certified on the front cover in blue ink, "Flown in Lunar Orbit for 6 days during Apollo 15, July 26-Aug 7, 1971, Dave Scott, Apollo 15 CDR.” The front cover also lists the part and serial numbers: “SKB32100115-312” and “1001." The covers are heavyweight cardstock boards, and the inner pages are lighter cardstock. This comprehensive checklist contains 141 checklist action pages (double-sided), including 22 red emergency pages. Contents include Power Critical Procedures (Lunar Orbit, Coasting, and Entry), Lunar Orbit Insertion (LOI) Aborts, Loss of Communication / Navigation, Contingency Jettison, Contingency EVA, P23 TLC Sightings, and Emergency Procedures. Major systems include charts, graphs, circuit breaker diagrams, computer procedures, and CSM systems emergency procedures. A handful of pages have brief handwritten notations in black ink. In fine condition, with staining to the back cover.

Accompanied by a signed letter of authenticity from Scott, who writes: “I hereby certify that the ‘CSM Contingency Checklist’ included with this letter was flown in lunar orbit for six days during the extended scientific exploration of the Moon and at the Hadley Apennine region during Apollo 15…Although the major objectives of the Apollo Program were to land men on the moon, explore the lunar surface, and return the crew safely to earth, the safety of the flight crew was of paramount importance. The stringent requirement for crew safety dictated the necessity of as much or more contingency planning than is provided for the nominal mission. Such contingency planning and procedures are accomplished by an adequate crew warning technique and a method of recovery. Crew warning was provided by cockpit indicators and Mission Control; and Contingency Procedures as presented in this Checklist were defined to ensure a method of recovery is available quickly…As the mission commander of Apollo 15, I was in charge of the handling, use and dissemination of the Flight Data File (FDF), as such, this ‘CSM Contingency Checklist’ has been in my personal collection since NASA presented it to me upon our return to Earth.”
