From the collection of Lunar Module Pilot Charlie Duke

Apollo 16 insurance cover with a cachet of the mission insignia and two sets of astronaut wings, bearing an April 16, 1972, Kennedy Space Center launch date postmark, signed in blue felt tip by John Young, Charlie Duke, and Ken Mattingly, with Duke writing “Insurance Cover” in the upper left. In fine condition. Accompanied by a photo of Duke with the cover and a signed letter of provenance from Duke, in part: “This…was signed by all three crew members of Apollo 16. It is labeled as an ‘Insurance Cover’ because it was to help provide for our families in the unlikely event we did not return from our mission…Since returning from the Moon, this cover has been in my possession.” Pre-certified Zarelli Space Authentication.
