Pension and Welfare benefit card is filled out by Sedgwick in black ink, printing her full name, “Edith Minturn Sedgwick,” her New York City address, birthdate and Social Security number. Signed at the bottom “Edith M. Sedgwick.” On the reverse, Sedgwick lists her mother, Mrs. F. M. Sedgwick, as the recipient of her death benefits.
