Vintage fountain pen signature, “3312 Blanchard Drive, Glendale, Calif. Jess Willard, Champ from April 5th, 1915 to July 4th 1919,” on an off-white 6.25 x 5 sheet affixed to a larger 10.5 x 6 sheet. In very good condition, with show-through, rippling, and staining from mounting adhesive on reverse, a central horizontal fold (lightly affecting a single stroke of the signature), and a small portion missing from lower right edge.
