Ashe, Jr.” Ashe also fills out the inside, printing his full name and adding his date of birth and gender, “Arthur R. Ashe, Jr., 10 Jul 43, M.” Lower portion indicates that Ashe was vaccinated for smallpox on November 30, 1974. Also included is Ashe’s Blue Cross Blue Shield health insurance card, 3.25 x 2, issued to “Ashe Arthur Jr” and sponsored by the AFTRA Health Fund. In overall fine condition.
