Twice-signed ADS, signed “P. Physick,” one page, 5 x 3.25, October 2, 1808. Bill issued to the Estate of William Byerly for “setting his fractured jaw bone and arm 8th Nov & for advice and attendance afterward.” Signed at the conclusion acknowledging a thirty-seven pound payment, with Physick adding his name again to the top heading.
