Vintage ballpoint signatures and inscription, “To my darling April, Jack Swigert,” and signed underneath by Tom Stafford, Ron Evans, Gene Cernan, Walt Cunningham, Donn Eisle, and Dave Scott on an off-white 8.5 x 11 sheet, with notation at top reading, “September 6, 1967 - Las Vegas, Presentation Arrangements.” Sheet also bears secretarial signatures of Alan Shepard and John Glenn.
