Interesting DS, signed “Walter E. Disney,” one page both sides, 3.5 x 6, International Certificate of Vaccination or Revaccination against Smallpox letterhead, no date. International Certificates of Vaccination, form PHS 731, opening to 6 x 14, printed by the Government Printing Office. Inside indicates that Disney received a smallpox revaccination on June 29, 1965, prior to his last overseas trip. Signed at the top in blue ballpoint by Disney, and also signed by his personal physician, F. Bergstrom. In fine condition, with three horizontal folds and a small staple hole.
