DS, signed “Jacqueline Kennedy Onassis,” one page, 8.5 x 3.5, no date, but circa October 1973. Parental permission form from the Collegiate School in New York, filled out and signed by Kennedy, giving her son John permission to purchase a school blazer. Neatly signed “Jacqueline Kennedy Onassis,” adding her address and filling in her son’s name, “John Kennedy.” In very good condition, with a bisecting vertical fold, scattered light soiling and toning, rough top edge and light overall wrinkling.
