Bergman has boldly signed the application in fountain pen and has also filled in the address section, telephone number, agent’s name, etc. and checked “under $25,000” for total radio earnings and boxes indicating her SAG membership, etc. Also noted is “$15.50” enclosed for dues/initiation. In fine condition, with faint edge toning, a horizontal fold touching the tops of a couple of letters of the signature, a stray ink mark affecting nothing. R&R COA.
