The front of the medal depicts the allegorical 'Head of Peace' above "St. Louis, U.S.A. 1904," and "1904 AAU Handicap Games, St. Louis"; the reverse is engraved with the name of the event, "Throwing 16 lb. Hammer." Suspended by a red, white, and blue ribbon from its bronze bar, inscribed: "1904 Universal Exposition, Olympic Games, St. Louis."
