Jack Ruby's unfilled prescription form filled out and signed by Dr. Perry E. Gross, one page, 4.75 x 4.75, May 15, 1956. Gross prescribes 50 pills of "Ambar," a diet pill consisting of methamphetamine and phenobarbital. In fine condition. Ruby would later tell prison psychiatrists that he had taken Ambar repeatedly in the past. In a 1964 interview with the police, Dr. Gross noted that Ruby was 'aggressive, impulsive, and impatient.'
