Accompanied by a detailed handwritten and signed letter of provenance from Lousma, in part: "The two Ascent Cue Cards that accompany this letter were both flown and used by the crew aboard the STS-3 missions during March 22–30, 1982. These cards…are used for quick reference and decision-making when failures occur in this 'dynamic phase' of flight."
